Instructions for Completing Adobe Acrobat (.pdf) Forms:
Open file with Acrobat Reader.
Click with the mouse in the first field to be completed.
If the field requires text, type the information in the field. Font size should adjust, if necessary.
If the field is a checkbox, hit the space bar or mouse click to check it off.
Use the tab key to move from one field to the next (including checkboxes), or click on appropriate field with the mouse.
Please note that you can't save the completed form to a file. You'll have to print it then mail or fax the hardcopy to us.
