Slot Tournament Registration

Please review the Slot Tournament information before completing this form.

Entry tickets will be distributed to the Team Representative at the team's first game on Saturday morning along with T-shirt orders and participation gifts. If other arrangements are necessary, please complete the Special Instructions box below. A confirmation Email message will be sent to the address provided with further information and instructions. Please make sure that the address you supply is correct and remember to update us if it changes between now and the tournament.
