TOURNAMENT ROSTER

Please complete this form for your regular league (CBO, CYC, JAO, NAU, or SEYO) roster players who will participate in the tournament.  The information you provide will be verified against the official league records.  Any changes to the regular league roster could affect your team's acceptance and/or division placement in the tournament.  Player additions will only be considered to allow a maximum of 8 players on the team roster.  Any additions made to the roster, once accepted, will be subject to the following charges per player:  $25 through the end of May, $50 during June, and $75 during July.  No additions will be allowed in August. All roster changes must be submitted on the Roster Change Form and are subject to Tournament Committee approval.    

Note:  For teams with more than 10 players, please complete this form with half of your players and click the Submit Roster button.
